Home Based Carers provide supervised care and support to patients and their immediate family’s/household members in the home and appropriate community-based settings in order to promote, restore and maintain a person’s maximum level of comfort, function and health.
The programme is divided into five modules covering the following:
- Home based care, communituy health care and social support
- Home Based care practices
- Holistic Assessment and Support in Household and Community Care
- Home-Based Care Implementation
- Referral, Follow-Up, and Documentation in Home-Based Care
The programme consists of:
- Facilitator led online knowledge modules
- Facilitator led online practical modules
- Workplace-based learning modules (56 compulsory workplace-based days)
- External Integrated Summative Assessment (exam)
